<span style="font-size: large;">In the third-floor ICU of SL Raheja Hospital in Mahim, 15-year-old Sagar (name changed) wants to play on his mobile phone, but is unable to speak because a cancerous growth was removed from his lower jaw recently. So, he uses a special app on iPad to ping the nurse station: "I want to meet my mother." When his mother is called in, he pings again: "I need my mobile phone." A few minutes later, he gets a mobile phone without a SIM card to play.</span></div>

<span style="font-size: large;">Simple tech solutions that are now making ICUs friendlier places—and helping patients communicate with their families as well as the medical staff—worldwide have just reached the city's shores.</span></div>

<span style="font-size: large;">The Vocalizer for Patients app that Sagar uses is the brainchild of cancer surgeon Dr P Jagannath. It has visuals with voiceovers for around 60 situations in ICU. "Many patients here have an endotracheal tube that takes away their ability to speak even though they are conscious. Many surgery patients too cannot talk on the first day after surgery,'' said Dr Jagannath.</span></div>

<span style="font-size: large;">"Some patients feel very cold but are unable to indicate so. Some others want to turned over or their back is itchy,'' he said. It is to help these patients that Dr Jagannath's NGO, Crusade Against Cancer Foundation, funded the project.</span></div>

<span style="font-size: large;">It is well-established that ICU patients suffer from ICU psychosis, said intensivist Dr Khusrav Bajan from Hinduja Hospital, Mahim. "Every effort must be made to help them communicate," he said, adding that many hospitals have devised methods such as pre-written notes to help patients.</span></div>

<span style="font-size: large;">Dr Sanjith Saseedharan, who heads the ICU at SL Raheja Hospital and has worked on the Vocaliser for Patients app with Dr Jagannath, said he plans to use four more iPads in his unit soon. "We began our trials six months ago with a 21-year-old patient, Sunil (name changed), who was diagnosed with Guillain Barre syndrome,'' said Dr Saseedharan.</span></div>

<span style="font-size: large;">Sunil, who was studying for his MPSC exams, suffered such an intense attack that he couldn't talk or walk. "I was in hospital for 75 days, of which 45 days was in the ICU with ventilator support. I could only move my fingers and my eyes,'' he told TOI. He became the first to use the app, communicating with his parents on the need to change sides or feeling cold.</span></div>

<span style="font-size: large;">Doctors at Bumrungrad International Hospital in Thailand have been using the IBM Watson Supercomputer system for about a year now, and oncologists at six hospitals in India started using Watson a few weeks back.</span></div>

<span style="font-size: large;">One of those oncologists is Dr. Amit Rauthan, who practices at Manipal Hospital in the southern Indian city of Bangalore. Watson is fully integrated into the computer system at the hospital, so soliciting Watson’s opinion takes a few seconds and a few clicks.</span></div>

<span style="font-size: large;">In an interview with STAT, he said he has used Watson for about two dozen patients. Most of the time, he said, Watson affirms that the decisions he had already come to were correct, such as one breast cancer patient who needed aggressive chemotherapy.</span></div>

<span style="font-size: large;">Other times, Watson disagrees, and the doctors change their mind. One of Rauthan’s patients had stage 2 colon cancer, and Rauthan thought that it would need chemotherapy. Watson recommended skipping the radiation and observing the patient — and its opinion won the day.</span></div>

<span style="font-size: large;">Rauthan emphasized that Watson itself didn’t make the decision. In the case of the colon cancer patient, Watson provided Rauthan with evidence about why chemotherapy might not be so beneficial, which he was able to share with the patient.</span></div>

<span style="font-size: large;">However the information about what happened to Rauthan’s patients — whether the chemotherapy helped the breast cancer patient, for instance — isn’t presently used to help Watson improve going forward, Kris said.</span></div>

<span style="font-size: large;">“That’s the ultimate goal — that it will learn from that experience,” Kris said. But that goal is years away, and one of the hurdles to achieving it is making sure that Watson can access and understand all of the information it needs about the patient — the complete medical record.</span></div>

<span style="font-size: large;">IBM announced last week that it was expanding the program to China, with clinical use projected to start next month. Part of the utility of Watson is to “democratize access to cancer care,” said Dr. Kyu Rhee, chief health officer for IBM Watson Health, reaching areas that don’t have easy access to information. Cancer is the leading cause of death in China, killing an estimated 2.8 million people in 2015.</span></div>

<span style="font-size: large;">Dr. Qunwei Chen, an oncologist in eastern China whose hospital is adopting the system, said he anticipated that Watson for Oncology will be useful, especially for doctors in small hospitals, where oncologists have less access to conferences at which they might learn about new treatment guidelines and procedures.</span></div>

If it can’t read the doctor’s handwriting, it doesn’t know what the doctor tried to do. So a key next step is making sure that the data feeding into Watson is really good, which may prove a challenge.</span></div>

<span style="font-size: large;">“The vast majority of patient data gathered and stored in electronic health records is totally unstructured,” said Zach Weinberg, president of Flatiron Health, a medical software company. One of the things his company is doing is trying to organize this data in a more useful way.</span></div>

Manipal Hospital has even taken the system one step further, allowing patients to seek a Watson consultation remotely. The person can upload their medical records, pay 9,500 rupees (about $140) and then receive a PDF report of Watson’s analysis of their case, verified by Manipal physicians, along with an e-consultation with a Manipal doctor. The report does not come directly from Watson, an IBM spokesperson said.</span></div>

Oncologists at Memorial Sloan Kettering Cancer Center trained the system to provide appropriate treatment recommendations by giving it descriptions of patients and then telling Watson the correct treatment.</span></div>

<span style="font-size: large;">Watson looks at records from a patient, like doctors’ notes and lab test results, and gives opinions about what treatment to pursue. Those opinions are backed up by easily accessible evidence — doctors can click on the recommended treatment and see which medical studies support it.</span></div>

When examining different patients, Watson agreed with Memorial Sloan Kettering doctors over 90 percent of the time in one study, and 50 percent of the time in another. These studies were presented at a 2015 oncology meeting, and the abstracts were published, but the studies themselves are not. Authors include IBM-affiliated scientists.</span></div>

<span style="font-size: large;">MAKATI, Philippines - Health tech startup Medix.ph has called in advertising executive, Lloyd Tronco, to pick up COO duties as the company forges into new prospects in the latter half of 2016.<br /><br />Best known as the company which provides electronic medical records for dentists, Medix.PH goes beyond its comfort zone of serving only dentists by launching out into other medical practices.  <br /><br />Tronco, whose career started out in various brick-and-mortar businesses including outdoor advertising, signage and billboards, and later on being employed as a Media Strategist at McCann-Erickson Philippines, has taken on the duties for operations in Medix beginning the second half of June 2016.  This will be an interim role as Medix seeks for a permanent operations head.<br /><br />"Now is a really good time to step in and steer Medix through uncharted waters", said Tronco.  "There are a lot of similar tech companies who are also involved with electronic medical records and each one of us are finding our respective space within the health tech ecosystem.  What I bring to the table to complement Medix's foray into additional practices apart from dental is the insight of what doctors and medical professionals really want out of their EMR", continued Tronco.  In this endeavor, his previous experience as an ad professional comes in handy.<br /><br />Medix is known as the company which provides the electronic record keeping for expansive multi-dentist clinics such as Gan Advanced Osseointegation Center (GAOC).  Last year, Medix also engineered the IT backbone of Ayala Health's Family Doc brand of community clinics.</span><br />

This will enable scientists to separate exosomes from body fluids and will be able to serve as trackers for biomarkers for cancer and other diseases.<br /><br />Cell membranes release Exosomes that contain genetic and protein information, these can be spotted in cancer cells also. This is important  with regards to the goal of developing liquid biopsies from blood samples.<br /><br />Deterministic Lateral Displacement (DLD) pillar array is used by the new IBM microchip. In fact, they’ve created a grain-sized sensor implant to prove their point.<br /><br />Engineers from the University of California Berkeley have designed a tiny sensory transplant the size of a grain of sand. When implanted, the device allows users to monitor the body’s vitals in real time. It can also be adopted to one day neurologically link individuals to their prosthetic limbs and assist users in operating them, reports Gizmodo.<br /><br />The sensor itself is approximately 3 millimeters long and is powered by a piezoelectric crystal. <span style="font-size: large;">Since the inauguration of President Duterte, the wave of “surrenderees” has been astonishing. The  number has been tagged at 500,000 as of August 1, 2016. These “surrenderees” are composed of drug addicts and small time pushers coming from the destitute communities all over the country. They are depicted in numerous TV news, radio programs, and print matter.<br /><br />It is unprecedented as was pointed out as an indicator of success on the new administrations’ war on drugs. But the sheer number overwhelmed the services that is supposed to serve as the first base of managing the illegal drugs problem. The local health units of every LGU, already devolved from the national agencies found it difficult to cope with the unexpected deluge.<br /><br />The Department of Health (DOH) has already been preparing for such an eventuality since “D-Day” or the first day of the new administration. In coordination with other government agencies on how to manage the influx of the surrenderees.<br /><br />Last August 2, 2016, DOH-NCR initiated a 4-day program in training about 30 doctors from local government units (LGUs) with regards to determining and diagnosis of drug addicts. This will also be the basis on what kind of treatment will be applied upon referral to the DOH.<br /><br />Dr. Alfonso Villaroman of the DOH treatment and rehabilitation center based in Camp Bagong Diwa, Taguig City cited the different levels of addiction during a health forum in Quezon City. The different levels will result in different treatment protocols ranging from counselling, out-patient rehabilitation or being admitted to a residential rehabilitation facility.<br /><br />Dr. VIllaroman explained the types and these are the following:<br /><br />1.    Experimental Users – those that tried using marijuana and then never used it again.<br />2.    Occasional Users – those that take drugs during parties and night outs with friends.<br />3.    Regular Users – those that imbibe on illegal drugs consistently (every 3 days, once a week or every 2 weeks).<br />4.    Compulsive Users – also diagnosed with co-morbid disorders or co-occurring diseases. The frequency of use will result in mental disorders (psychosis, paranoia), hypertension, cardio-pulmonary diseases and even infectious diseases such as HIV and TB made more likely because of the lifestyle. <br /><br />The participants of the training program will be able to assess the level of addiction and will recommend the correct response to each addiction.<br /><br />Levels 1 to 3 can be managed by counselling and out-patient programs while those level 3 and 4 will be managed by admission in rehabilitation centers. <br /><br />“Addiction is an illness. Not all illnesses are severe. It’s the same with addiction.” Said Dr. Villaroman.<br /><br />LGUs will also be provided a manual for community based drug treatment that was launched last year by the DOH and the United Nations Office on Drugs and Crime together with the Dangerous Drugs Board (DDB).<br /><br />This training will also be given to social workers of the DSWD and will be adopted in other regions. This multi-pronged approach will be utilized to manage the “surrenderees” and is part of a broad spectrum strategy in fighting illegal drugs and drug addiction in the country. The “three pillars” of this approach will be “assess, diagnose and refer”. This will be undertaken with the cooperation of the DILG, DOH and DSWD. <br /><br />Dr. Villaroman said “Don’t be judgmental. They can be the best that they can be.”</span></div>
